Admissions & Enrollment
Step 1: Fill out the Registration Form to reserve a spot.
Step 2. Prepare the following requirements.

1 photocopy of Form 137 or Original Transcript of Records

Certificate of good moral character or barangay clearance.

Medical certificate (fit to study)
-
1 photocopy of Birth Certificate (NSO/PSA)
-
4 pieces of Passport size photo with name tag (on white background and with collar)
-
4 pieces of 1x1 photo (on white background and with collar)
Step 3. Submit your requirements in person.
Note: No admission shall be made if records required for enrolment are incomplete. In exceptional cases, the trainees shall be considered temporarily enrolled only for the first week of training pending the submission of pertinent records. Compliance with admission requirements must be the trainee’s priority.
